Dr. Mistry explains that the penile prosthesis was invented by Dr. Brantley Scott of Baylor College of Medicine in Houston (the same medical school Dr. Mistry attended). This remarkable and life changing device uses a manual pump to inflate a silicon implant with fluid from a reservoir--all completely concealed within the patient's body.  NAU Urology Specialists' own Dr. Christopher Yang spent an entire year training to perform this surgery. Dr. Mistry: Hello and welcome to the Armor Men's Health Hour. I'm Dr. Mistry, your host, here as always with my effortless and very helpful co-host, Donna Lee.
Donna Lee: Thank you. I'm always wondering what that word is going to be to describe me.

Donna Lee: You can't. And you sound silky smooth today, so that's good. Good start.

Dr. Mistry: Well, we're still stealing their , their stuff.
Donna Lee: That's true. We're still stealing their bits.

Dr. Mistry: I love it. I love it. Your questions are what drive this show. We really appreciate you sending us questions. And the patients that come to our practice also inspire many of the topics that we talk about. But, but today I think we're gonna do some questions, right Donna?
Donna Lee: That's right. We have a question that was sitting in a while back. So he's been waiting for a response. So this is a good one, saving it just for you.
Dr. Mistry: Good for you.
Donna Lee: This gentleman, "I'm a 67 year old male type two diabetic. And I've struggled with ED for years. I used pills years ago and they progressively got less effective. I'd tried a shockwave type therapy, but did not have any luck with that. I went to the Trimix in injections, which worked , but it is almost impossible to orgasm with the Trimix injection. The erections don't seem to be as firm as they once were. My family doctor had me on one CC of testosterone every two weeks. With all that said, I'm now thinking about a penile implant." Very nice decision. "I know there are pros and cons with this procedure, but the satisfaction rate from what I hear is very high. I guess my question is the next logical step in what's the next logical step in my quest for satisfying sex life? Thanks so much."
Dr. Mistry: It's a great question. And it really shows the open-mindedness that many patients have towards potentially a surgical intervention for their erectile dysfunction. Erectile dysfunction for each individual man impacts them differently. You know, I'll see some 65 year old men who haven't had sex in 20 years. Some guys that can't live without it, if they don't have it twice a day. And others that find it...
Donna Lee: What?!
Dr. Mistry: Donna, it's just you.
Donna Lee: Sex twice a day? Shoot me.
Dr. Mistry: It's just you. Oh my Lord. It's just you.
Donna Lee: How about twice a month? Twice a year?
Dr. Mistry: Twice, twice a month? Michael got really excited all of a sudden.
Donna Lee: Yeah I could hear him through the radio.
Dr. Mistry: Is it my birthday twice a month? So yeah.
Donna Lee: Twice a year.
Dr. Mistry: So the penile prosthesis is a procedure invented at Baylor College of Medicine. Some of you may have heard of it. It is the finest institution ever invented.
Donna Lee: No. Stop. It's kinda where you went.
Dr. Mistry: ...the top urologists in the , in the, in the universe. When the aliens come here gonna be like, "I would like to go to a Baylor-trained urologist." And so it was invented by Dr. Brantley Scott at Baylor College of Medicine. And you really have to give it to the guy. He saw a flacid penis and was like, "You know what?"
Donna Lee: "Let's stick something in there."
Dr. Mistry: "I'm going to stick a silicone implant in there and you're going to be able to pump it up using fluid that comes from other reservoir." I mean, the engineering that went into it is remarkable. All the components are inside your body. It's made, all the components are usually installed, I should say, through one incision. And Dr. Christopher Yang, one of the partners here at our practice, spent an entire year doing nothing but putting these in.
Donna Lee: Wow. That's a lot!
Dr. Mistry: In Florida!
Donna Lee: Right?
Dr. Mistry: You know what I'm saying?
Donna Lee: There's no...
Dr. Mistry: The center of, you know, 65 year olds with a erectile dysfunction.
Donna Lee: There's no loss of penises that need it there.
Dr. Mistry: That's right. And so the, you know, the best part is? It's covered by insurance.
Donna Lee: That angers me.
Dr. Mistry: You really have to give it to these guys.
Donna Lee: Y'all get lucky with everything.
Dr. Mistry: Well, you know, except for the erectile dysfunction, we're very lucky.
Donna Lee: That's true.
Dr. Mistry: And so, and so it's covered by insurance. It is a very high satisfaction rate. Over 90%. It allows you to obtain a spontaneous erection whenever you want one by simply pumping up a little pump that's in the scrotum. The reservoir pump and the penile cylinder is all contained completely within , within yourself. You can't tell that you have this when you're just looking at yourself normally. And for the most part, you can't really even tell if somebody is touching it. As a physician, I know what it's not supposed to feel like. So I know that you have one. And the recovery, some physicians do as an outpatient. Generally , we like to keep you overnight for a little extra dose of antibiotics. If you're a diabetic, the biggest risk that you have to worry about is the components getting infected, but there's special coatings on it. We give you antibiotics for it. And generally the infection rate is very manageable. And I think that if you have advanced erectile dysfunction and haven't been given information on the inflatable penile prosthesis, you're really missing out.
Donna Lee: That's crazy. So you can't tell? So if I'm the wife, I just can't tell that my husband's got a...
Dr. Mistry: It depends on how often you touched it before, beforehand, you know what I'm saying?
Donna Lee: Well, let's refer to a normal marriage.
Dr. Mistry: When I , when I was at the VA at this guy came in and said that he needed to get his taken out because his wife kept activating at night and jumping on him.
Donna Lee: Stop it.
Dr. Mistry: I'm not kidding.
Donna Lee: Are you serious?
Dr. Mistry: Absolutely.
Donna Lee: So he wanted it out?
Dr. Mistry: No molesting! And so , that was a , it was a funny request. And then the other question that this, the caller had that I think was kind of overlooked is this issue of not being able to orgasm with Trimix. And so...
Donna Lee: Tell everybody what Trimix is.
Dr. Mistry: Trimix is a combination of three different drugs that are injected right into the penis. Now that sounds really remarkably invasive. But in fact, the needle is tiny, the medications work like a charm, and it works extraordinarily well. Like the combination works very well.
Donna Lee: So the little needle just feels like a little tiny prick? Pun intended.
Dr. Mistry: Hey! And so it's a little prick. And so...
Donna Lee: It makes a big prick!
Dr. Mistry: It does! We should go into marketing. And so when you, when you use this to get an erection, sometimes you can get too strong of an erection, too much of a blood flow into the penis. And that can inhibit orgasm . So if you're out there using Trimix and you're not able to achieve an orgasm, come on in and let us kind of tinker with your , with your tinker...let us tinker with your dosing so that we can see maybe one of the three components is , there's too much of it. And usually it's Prostaglandin. And if there's too much Prostaglandin in your mixture, it might inhibit your orgasm.
Donna Lee: What are the three ingredients?
Dr. Mistry: No, that's not fair. Prostaglandin, Papaverin, and...
Donna Lee: I'm not trying to trick you.
Dr. Mistry: Phentolamine.
Donna Lee: And the other one . So I didn't realize that you could change the combination amount on each.
Dr. Mistry: That's right.
Donna Lee: I always thought it was all the same all the time.
Dr. Mistry: We have 15 different formulations here. Look at the forms once in a while, lady.
Donna Lee: I don't, I'm not clinical.
Dr. Mistry: Okay, well...
Donna Lee: I just put out fires, and...
Dr. Mistry: She's just Dr. Donna!
Donna Lee: Dr. Donna doesn't need clinical information. She needs a degree!
Dr. Mistry: Right? So, but if you're in, but if you are at the stage in which you haven't tried the Trimix injections , you know , I would encourage you not to go to one of these fly by night men's health clinics out there that advertise this stuff for $2,000 a month. I mean, the medication shouldn't cost you more than $5 to $7 a dose. It's available widely all these medicines, all the components of it are generic. There's just no reason for these things to cost you a lot of money. So if one of the major hangups you have for addressing your men's health issues is concern about insurance covers and money, I mean, it's definitely something that at least in our practice, we really to value your cash and make sure you get the best care for the least amount of money. I mean, $7 to $10 a dose for the Trimix, the penile prosthesis is covered by insurance. We can easily get pills for less than a dollar a dose. I mean, this is testosterone for less than $30 a month. I mean, these are the kinds of things that we want to make sure aren't barriers to your health.
Donna Lee: That's fascinating to me that it's covered by insurance because women can't get boobs covered by insurance, but you guys can get your penis is covered by insurance.
Dr. Mistry: Well, you know, if you have breast cancer, then the breast augmentation and all that stuff is covered by insurance.
Donna Lee: That's covered by insurance? Yeah . But I mean, just not like the guy doesn't really need a prosthetic penile implant.
Dr. Mistry: He is not functioning well. Of course he needs it for his mental health. And it's really a gift that keeps on giving. It's for his wife, too.
Donna Lee: Except for that one guy who wanted it taken out.
Dr. Mistry: Well, you know, he was being abused.
Donna Lee: He was being molested by his own wife at night. Oh my gosh. That was a good one.
Dr. Mistry: I wish Michael had that problem.
Donna Lee: He does, too . He said, "I want to just travel with my wife one day." And I was like, "I hope she's nice, good luck. I'm stuck."
Dr. Mistry: Very good.
