In this segment,  Dr. Mistry explains why male enhancement interventions can be as much about function as they are about aesthetics. Although many men have concerns about how their penis looks, they are often reluctant to ask their urologists about options for penile enhancement for fear of seeming vain. Unfortunately, this fear is not unfounded as some urologists and other health care providers do not take "cosmetic" concerns as seriously as they should. In reality, questions about male enhancement options are often indicative of other, more serious conditions. For example, a man suffering from buried penis might only have "superficial" concerns now, but if this condition goes on long enough, he could end up like one of Dr. Mistry's recent patients who has an almost entirely inverted penis and was unable to be catheterized by ICU staff. Dr. Mistry explains that all urological issues are important and patient inquiries should be taken seriously. Dr. Mistry: Hello and welcome to the Armor Men's Health Hour. I'm Dr. Mistry, your host, here with my incredibly funny, very knowledgeable, very helpful, and technologically sophisticated--one of those things is not true--co - host, Donna Lee.

Donna Lee: Well it's certainly that I'm funny and I'm super cute, but...

Dr. Mistry: I did not say anything about you looks at any point.

Donna Lee: I know. I had to insert that. But technologically unsavvy is my official title at work now.

Dr. Mistry: I am a board certified urologist. This is a men's health show.

Donna Lee: No. And why does it cost a hundred thousand dollars to do anything when you've got, when we want to make a website change. And you're like, we have to take out a loan from the bank, sign paperwork...

Dr. Mistry: Stop complaining. Anyway, what we do around here is urology. Not really good websites.

Donna Lee: Or podcasts.

Dr. Mistry: That's right. We have amazing providers in our practice. We are the second biggest urology group in Austin. That means that we just try a little harder.

Donna Lee: ... try a little harder. Do a little push-up on the weekend. A little more lipstick.

Dr. Mistry: And I think it's because we can't, very hard to scale up the incredibly personalized care that we try to offer patients. We have high standards for what we want to do for you when you enter our practice, high standards for ourselves. We really want you to well taken care of. So if you're somebody that's looking for just a quick answer, then this may not be the place because we want to give you a good answer and restore your function and get you feeling great.

Donna Lee: And a thorough answer.

Dr. Mistry: And a thorough answer. So Donna, how do people get ahold of us and get to know us as a patient if they want to see us?

Donna Lee: That's right. You can call us. I had another patient very recently actually call and he said he was listening to our podcast and he really likes your approach to holistic urology. And he's been going to the larger group in town, and that he said...

Dr. Mistry: There's only one.

Donna Lee: There's only one. They did not, at one point, did they ever ever ask him about what he eats and his nutrition. And he said that once he heard their , our show, and then he, then it bothered him. And he said it didn't bother him until he realized that they weren't asking the right questions . So we're going to ask you all about your business.

Dr. Mistry: Absolutely. I appreciate that. How do people get ahold of us?

Dr. Mistry: Dripping in Dripping. You know, to your point, we often see men with fertility problems. A lot of times they wonder why we delve so deeply into issues like nutrition and diet and overall health and sleep. And I try to explain to them that if you were designing the human body.

Donna Lee: ...or a Ferrari.

Dr. Mistry: Or a Ferrari. No, if you're designing a human body and you were saying, "Well, who should have the best sperm quality? A real healthy person, or kind of a sick person?" You'd say, "Well, the healthy person, because they should be reproducing." And so if you're overweight and have sleep apnea with pre-diabetes and high cholesterol, don't be surprised when your sperm counts aren't so great. So the sperm count is both a barometer of your health, but also something you can work towards and try to, and by making yourself healthier, you can make your sperm counts better. So it's an example of how we use kind of your body and your urologic condition and combine the two, two issues to kind of motivate you to do better. And then we don't just say, go lose weight and lower your cholesterol. We give you a resource. We have an integrative nutritionist on staff who's going to work with you and make things better for you. We'll prescribe you medicines when necessary. I mean, we're here for you. We'll work with your primary care doctor to make your numbers better. That's what we're here for. And so I just wanted to make sure that our listeners knew kind of why we take that approach because it applies to every disease process.

Donna Lee: Well, everything's connected. The neck bone is connected to the knee bone, and the wrist bone's connected to the tummy bone.

Dr. Mistry: Sometimes you can know how to sing and sometimes you don't. But you know, I'll tell you one bone that we worry a lot about.

Donna Lee: The pee pee bone?

Dr. Mistry: The main bone. Yes. And we, our third, most downloaded podcast is when we talk about male enhancement surgery. And I've told the joke on the radio before that one of the first marketing campaigns that I ever did was about the FLP, the funny looking penis, and that's whether you have warts on it, whether it's curved, whether the scrotal skin is on there too, too, too much and it gives the penis a buried sensation. And I probably see at least four patients a week now who are looking to get...

Donna Lee: A bigger penis?

Dr. Mistry: Well for many reasons. And so on its surface, it may seem completely cosmetic. They just want a bigger one. But that's not the only, that's not the only kind of rationale. Unfortunately, last week I had a patient that I had to see in the intensive care unit who had a severely buried penis. He was a very heavy diabetic man on a , on a ventilator in the ICU, and they couldn't get a catheter in him because his penis had inverted and buried itself into a skin.

Donna Lee: How did he potty ?

Dr. Mistry: He just dribbled.

Donna Lee: Oh, so he had to wear a diaper?

Dr. Mistry: No. He probably sat on the toilet and dribbled, and so his scrotum got wet. But in this particular case, he wasn't able to potty because he was intubated. And so they needed a catheter, but nobody could get into, into there and they couldn't find his penis. So that's a severely buried penis. And if there are, but that guy knew he was like that before he got sick in the ICU.

Donna Lee: Yeah, that didn't happen overnight.

Dr. Mistry: He just, he knew the penis was getting smaller and smaller, going further and further into him, skin getting tighter and tighter, and he's dealt with it. And then I had to go and at, you know, three o'clock in the morning and try to cut my way into this skin thing.

Donna Lee: Oh my heavens.

Dr. Mistry: And, and I , you know, I thought to myself, you know, a lot of times when I talk about buried penis or penile enhancement, you know, people may turn off the radio thinking that it's just something to do with cosmetics, but there's a real functional component here. So if the fat pad above your penis is too big, or you had a circumcision later in life and the too much skin got taken off, or your scrotum, a lot of times the scrotum's real big as you get older, can obscure the size. And if you're a diabetic and you're real heavy and your penis is going into you, you need to come see somebody, because we'd rather take care of it in an elective fashion when we have lots to work with lots of tools, lots of skin, lots of, you know, consultants than emergently, you know, in the ICU or an emergency room when somebody tries to get a catheter in you. And like so many other patients, this particular one also had a stricture or a narrowing of his urine tube . So after working so hard to finally see the penis, when I tried to put the tube in, it wouldn't go, it wouldn't go in because he had a stricture inside there. So, you know...

Donna Lee: What's a stricture?

Dr. Mistry: A stricture is a narrowing of the tube that you pee from. It's like a scar band. And so I couldn't put a catheter in. I'm sure he was unable to urinate , which is really unfortunate. And so we had to open up the stricture kind of while we were working and finally got the catheter into him. So , a win from that fashion. Unfortunately , he had COVID was on , was on a respirator and didn't do so well, so...

Donna Lee: Well he's doing well now, right?

Dr. Mistry: He is not doing well now.

Donna Lee: Oh. Okay. Dang it.

Dr. Mistry: And so , so when it comes to kind of the way your genitals look, if it's in any way interfering with your function--if you can't pee right, if you can't hold your penis to pee, for example, because it's too far inside you...If you're concerned this week earlier, I had a patient who was constant concerned about his shrinking penis. And when I examined him, it was clear that he had very low, like his test , his testicles were atrophied and he had, I'm sure he had extremely low testosterone leading to this genital atrophy. So it could be a sign of something more significant. The repair sometimes it's covered by insurance, sometimes it's not, but we can always at least address it and look at it and see if it, if it makes sense. But taking that concern to any general urologist or any primary care doctor, a lot of times, they're going to feel like you're doing it for cosmetic reasons. And so you need to see somebody that's, you know, actively coming out to you to say, "Listen, if you're concerned about the appearance of your genitals, have somebody take a look at it, and have somebody that can fix it." We have exceptional general surgery partners. We work with Dr. David Dellinger , Dr. Jeffrey Cone, a number of other genitourinary plastic surgeons in town. We're going to have a couple more on the radio in a few weeks. People that take your concern seriously and can give you a solution that works to your benefit. And so if, if you're out there, if something doesn't look right, something doesn't function right, you should let us know.

Donna Lee: That's amazing. Now I feel so bad for this patient. Bless his heart.

Dr. Mistry: Well bless his heart.

Donna Lee: So if , I guess the moral to the story is if you can, how you said, if you can hold your penis when you're peeing...

Dr. Mistry: That's right, or if you feel like your penis is like going into your body further and further.

Donna Lee: Hmm. That just sounds so awful. Is it painful?

Dr. Mistry: It can be, but usually not. Or else they would show up sooner.

Donna Lee: Oh, I guess so , because it's so gradual?

Dr. Mistry: Yeah, but if you've had recurrent infections of the foreskin, if you, you know, if, if...but I have to believe you, you know, you guys know out there when something's wrong. Okay? And if you're not missing it, your , your wife knows, for sure.

Donna Lee: I don't know. She could be like me and be running away from it and not have any idea.

Dr. Mistry: She'd never seen it in first place. That's right.

Donna Lee: I don't know if Michael has a buried penis.
