Speaker 4We have a really interesting, very long question. This person didn't want us to know anything about him , even though these are anonymous when you send us these questions. But he listed his name as a cartoon character. Excellent. And then he has a fake email address as a response <laugh> . And I think he was like really wanting to give us
Speaker 3Good . Well , I , I I , I , I don't know how we're gonna tell him that we're airing the show, but let's, let's go ,
Speaker 4Let's go for it. Exactly . Well, he starts off a little aggressive. You need to talk about the different types of implants like Coloplast and Boston College and the difference. I'm a diabetic and I had the Coloplast put in. I preferred the Coloplast because it stretches and remembers. So you get bigger all the time. Insurance covers a hundred percent. That's kind of a weird statement because sometimes it doesn't. But anyway, important to note, if you have surgery, you need to get a penis pump and pump your penis up and stretch it out for about a month before the surgery so they can put it in the largest penis implant they can. So you can have to stretch out your penis before surgery. I don't think English was his strong suit, but I appreciate it. He mentioned the doctor who did this, which we will not mention. He said he stuffed a hue implant in me. It's big. And women don't know I have it. Women say how big I am and I can go so long. This is really interesting. I love itm . Sorry .

Speaker 4I think he's, it's both. He is bragging and has a question. Also, testosterone shots are not the answer. I was on testosterone shots and my hair started to thin and my penis go smaller and my body stopped producing testosterone because of the shots . You're

Speaker 3Don't do it. I said,
Speaker 4You not talking me anymore . Li said, I got off testosterone and started taking N clomophine 20 milligrams at night. And to have the sex, he said to have the sex. I love it. Anyway, he goes on and on about medications. I also have 75 to 80 units of HCG in the stomach once a week. Testosterone levels are much higher than they were. I take tline. Yeah . Said that correctly twice a week. I also take over the counter supplements, and then he ends with I can beep like a grand champion for hours and the girl be on birth control. If not, she will get pregnant. Exclamation

Speaker 4I don't , I think it was just getting a little ghetto at the end. Okay . Because I was like , you

Speaker 3Aggressive . You want me to just talk about the different points that he is talking about? There's so many points. There's
Speaker 4A lot going on in there. So
Speaker 3Let's love it . Love it. Let , let's talk about kind of this trend toward a different kind of testosterone supplementation program. So, mm-Hmm . <affirmative> , we do lots of hormone replacement in our practice. We have probably all of our practitioners to some degree can do it, but we have some practitioners that do advanced hormone replacement. So if you're out there and you've just been getting testosterone shots or testosterone cream and wondered, Hey man, I wonder if there's a science to this. There is certainly a very deep right and thorough science to it. So this particular patient mentioned three different drugs that they're on, all of which we use in our practice for different reasons. Mm . So the first one was n clomophine, which is a different, what we call isomer . Isomer or chemically. The, the way the the , the molecule is arranged is a different one than ch clomid or clomophine, which we use very frequently. Mm-Hmm. <affirmative> . It is a medication that's available through a compounding pharmacy. Is it , it costs about expensive , about a hundred dollars a month. Some men report that the impact of it is much pure and gives them a much better response. The key to it is it does not replace your body's machinery to make testosterone. It doesn't, it doesn't suppress you. Oh, I see. It uses your body's machinery. Oh, so it just juices you. It's a turbo. What's fantastic Turbo ? It's a turbo. It's a turbo. It's not, it's not switching your gas motor for a electric motor. It is, it is just supercharging what you should be producing on your own. Okay. Similarly, HCG injections, which he's reporting. Also use your own body's machinery to make more testosterone. So both of those can be used along the shots or instead of the shots if the shots weren't doing anything for you. Okay. And we try to personalize that the cabergoline we use in the practice, because that decreases a hormone in your brain called prolactin. Mm-Hmm. <affirmative> and prolactin, if it's too high, causes your testosterone to drop. But in our practice, the most important thing that it does is it reduces your libido. Oh. And what this guy really wants is a high libido. Yeah ,

Speaker 3If , if I can tell anything from the message and from his letter, it is that he really prides himself on having a very active sex life. Yeah, he does. And he enjoys his very high libido.

Speaker 3Well, again, we love questions <laugh> . It really drives our practice and , uh, and , and , and the show. And so let's talk about , uh, the next part, which is that he's a diabetic and has a penile implant. Mm-Hmm . <affirmative> , which we talk about on this show, Dr. Christopher Yang , uh, and Dr. Twain Nguyen . Those are two , uh, of our practitioners, both of whom do lots and lots of prosthetics. Mm-Hmm. <affirmative> , understanding the role of an inflatable penile prosthesis in your care for erectile dysfunction is important. So I, I jokingly tell my patients that if there's a , you know, if there's a will, there's a way, meaning if you have the desire to have sex, then we can tape something together to make sure that you have a hard enough erection , uh, to be able to accomplish the goal. And so , uh, for those that have heard this question and are still listening, <laugh> , um, I will tell you that there is, there is a procedure in which we can replace the internal hydraulic workings of an erection Okay . With either an inflatable device or one that stays semi-rigid all the time. Okay . Uh , for those with poor hand dexterity, you do have to pump up the device through a small pump that's in the scrotum. Mm-Hmm. <affirmative> , there's fluid transfer that occurs to these cylinders that then give you an erection. Some of the points that this, that this listener makes, which I think are really important to make Mm-Hmm. <affirmative> , is that the bigger you are already, the bigger the implant that can go into you is Oh,
Speaker 4Makes sense. There
Speaker 3Is no inherent difference between the Boston Scientific prosthesis and the Coloplast prosthesis in terms of its ability to get longer and fatter, like both of them. That's the whole purpose of the inflatable penile prosthesis that you want them to get longer and flatter. Right. Uh, in fatter, excuse me. So, so the , you know, I just don't want any listener to think that there's a difference in the, between the two companies devices. Oh, I see . What there is, is there's a difference between one that you pump up and one that stays up. Mm-Hmm . <affirmative> . And , um, the one that stays up, you know, it just kind of, it's like Gumby. It's just like, it just , you just fold it up when you want to use it and fold it down when you don't want to use
Speaker 4It. And the sensation's still the same.
Speaker 3The , your orgasmic sensation should remain the same. Uh , but what
Speaker 4About the senses that are along the shaft of the penis?
Speaker 3You should feel it from a skin standpoint. Now, nothing is unfortunately gonna feel like the, the original, so you're not going to get the engorgement, you're not gonna get a lot of the tingly feelings that you once got. Oh . Which is where, which is where sometimes concomitant use of Viagra or Cialis, which will improve that blood flow to the skin. Mm-Hmm. <affirmative> could sometimes be used even if you had a penile prosthesis. So his advice to use a penile stretching device or a vacuum erection device, I think is a very legitimate one. Mm-Hmm. <affirmative> , I don't know how much extra length it's gonna give you, but it will give the blood, it'll give the actual organ, it'll remind it what, what it felt like to be engorged, you know? Oh . You know , if if it , if it , if you , if you don't use it, you're gonna lose it. So a natural shrinking a natural kind of, you know, constriction of the, of the girth is something that I could certainly , uh, anticipate. So by getting back into a vacuum erection mode, I think that that is something that will help. Now many of our patients are already on what we call intracavernosal injection therapy. Mm-Hmm . <affirmative> or are already using a vacuum device. So the progression of doing that prior to an inflatable penile prosthesis is already part of our algorithm. Mm . But I could , I could certainly see somebody that has been completely flacid for several years may need to work it out so that we can put the right thing in you. Mm . Speaker 4That's what happens when you disappear. I , I need to have you back more often, but I have a quick little tidbit to tell you. So , uh, Valentine's Day , it's a very important day of love, you know?

Speaker 4V day , yes. For Valentine's, not the other, like venereal diseases. Oh, that too . Yeah . Although we can help that too. But you know, what I found out is Valentine's Day, and this is not a joke that we've made up, it's actually also National impotence day.

Speaker 6First of all, that's kind of a bad word, you know, impotence just seems like you're so powerless, you know? Well,

Speaker 4Know? Well, you know, we've been joking about it around the clinic, because ever since I found this out, I was like, well, we have to do something about this for the peoples.
Speaker 6You know, it's like Groundhog Day also around this time of year because, you know, like that's the one day we all count on sex and it's just amazing. That's not like Groundhog Day, but that's also time I count on sex. You have sex

Speaker 6Day , and , and then I have six more weeks of sex. No , it's not, it's not, it's like that movie , uh, with Bill Merck . I saw my shadow. Um, yeah, I saw other things <laugh> . Uh , but yeah , we gotta help people out, Donna, that's not cool. Um ,

Speaker 6Well, thank you. I , I think I, here's a story I tell people, first of all, okay . Um, it's not a story. It's more of a long diatribe that lasts just short of 11 minutes <laugh>, but it before

Speaker 6Effectively, I, first of all, whatever age we are as men Mm-Hmm. <affirmative> is a percentage of men our age who suffer from erectile dysfunction, which is the politically correct term for impotence <laugh> . So the bottom line is
Speaker 4Erectile dysfunction. Right. So we've never talked about that before.
Speaker 6No, but it , but you know what I mean, like, if, if you're 50 mm-Hmm . <affirmative> , which I happen to be at this airing of this tape. That's right. Um, it's 50% of men our age. So about half my friends are coming up to me going, Hey man, you got some samples that I can try? So the blue pill samples. The blue pill, sure. Mm-Hmm . <affirmative> . So, so that's kinda the way I think about it when I, first of all , um, wanna make sure there's nothing serious going on with you if you're a 30 or 40-year-old. Yeah. You have a 34, 40% chance of having ed. Mm-Hmm. <affirmative> . Um, but it could be pretending a sign of bigger problems like a heart problem , um, or an issue with , um, with your nerves or something going on with you. Mm-Hmm. <affirmative> . So you have to understand how erections happen. You have to have a nerve that fires, that makes arteries open, that brings blood into the penis. Mm-Hmm. <affirmative> , then your veins expand, and then veins stay, you know, dilated for a while . Then you climax the veins contract and then you go fla it and go back to your day to day routine. Mm-Hmm. <affirmative>. Um, so at any point, what I didn't mention there was testosterone. You know, you don't need testosterone to have an erection. It helps ,
Speaker 4I think. Yeah. I think that's the overall myth. People think the guys think they get on testosterone, they're gonna have hard erections again. Right . Immediately.
Speaker 6I , it's really just praying that's PREY .
Speaker 4That's happen . It does happen though, with some people. Sure.
Speaker 6And , and , and it can help, it certainly can help, it makes the desire the libido, but a castrate eunuch can have sex. They just don't want to. Mm-Hmm . You know, it's a reflex in their case. <laugh> . So, so yes. We don't want to minimize the , the benefit of testosterone because it absolutely helps, but it doesn't cause an erection. You gotta have nerves that fire and arteries that open and veins that expand, period. So, so then again, how do I approach that when I talk to my patients? Um, I, I like to think selfishly, what would I want done on my penis? Oh. And well, that's a whole other hour. You know what I'm saying ? Party there . But , but it is Valentine's Day. That's what anyone's listening, <laugh> . Um, so the point is, I am a very , um, practical, and B very cheap. And I'm thinking, and , and , and c I'm very risk averse and pain averse. Mm-Hmm. <affirmative> . So I'm thinking if I can take a pill Mm-Hmm . <affirmative> , I'm gonna take a pill if I need it, you know, and then you got Viagra and Cialis , uh, that worked very similarly, but also they metabolize a little differently. So Viagra is your, your quick in out Navy seal, get in, get out, quit messing about to use it . The politically correct term there. And
Speaker 4The Cialis is like a daily,
Speaker 6The Cialis is more of a, I wanna take it on a Friday. Uh, it's, think of like the good covid curve, you know, nice and flat. Like Fauci wants it nice and flat. Right . Lasts a long time. You can take, see Alison a day before Valentine's Day. It'll still be in your system. It just won't hit as high peak. It won't hit as high a big peak. Oh , I see . So some people need that high peak. Right. Some people just have to be planners though. Like, I want the biggest bang for the buck, if you will. I totally intended that button , by the way. No , you , thank you. Uh ,

Speaker 6Patients . But the, if you can, if you need to get that high peak, take the Viagra 30 minutes on empty stomach if you can, Hey, I want spontaneity. Mm-Hmm . <affirmative> . And then you say , if I want it more than twice a week. Mm-Hmm . <affirmative> . Well then you take a daily pill because mathematically five milligrams of Cialis daily is 35 milligrams a week. Trust me, I did the math on my feet. <laugh> . Um, 20 milligrams Cialis as needed is two pills, 40 milligrams a week. Mm-Hmm . <affirmative> . So if you're having sex once a every Valentine's Day, you don't need a daily pill. Right. You know , bottom
Speaker 4Line . That's true. Yeah . We talk a lot about the daily s um, on other episodes, but what , um, I, for we , I know, I know the side effects and the, the rules before Viagra, but I don't think the commercials say that you have to not eat four hours before. Is that right? How
Speaker 6Many are at least, at least 30 minutes on empty stomach ? Or like 30 minutes of onset? But yeah, Viagra is mess whatever it takes for your stomach to be empty.

Speaker 6I , again, being financially , uh, minded, I, I'm gonna go with a vacuum pump next on a , because I , I , it's 130 bucks. If you get it from us, you get a decent one. You can get a right . A terrible one at your local drugstore or

Speaker 6Yeah. Oh , the , if you Google it, it's gonna be like all sorts of weird sex shops and it probably doesn't have the V vacuum spelled correctly. So <laugh> , so , so, and then the rings that come with it look like something. I mean, you could get 'em at a sex shop if they were such a thing. I don't think there are anymore. Oh no. But, but you know, again, I think you get what you pay for, but you get a decent one for vacuum pump. And again, we're just trying to bring flow in and keep it there so you pump it up and put a ring at the base to keep it there. Um, for those who are married or in a serious long-term relationship, it works for those who are dating. It's kind of hard to take your Austin Powers device to your date. Right? Like , oh , what's this doing here, baby? I don't know. <laugh> <laugh> . Uh ,
Speaker 4So tell really quickly tell people how it's used. 'cause I, I recommend to patients that they learn from a urologist or a provider physician how to use a vacuum erect erection. Yeah.
Speaker 6This is not a Hoover. If there's a CD ROM that goes with it, and we , we have d displays demos in the office. Right . To show it to people. And again, just to make sure they're comfortable with the idea of what this thing actually looks like. There's a little mechanical hand pumps that you can use , or an automatic one that use the battery button battery and it costs a little bit more for the, for the , um, you know , automatic one. Right. But again, not for everybody. Those who like it say, I just take it, buy it one time. I don't take a pill every time. I just put it on when I want it, take it off when I don't. Oh . Um , the , if you don't like it, you don't , it's a tourniquet. It's too sex shoppy. It's too weird. And again, not for the dating couple. Right. So, you know , that brings us to what if that doesn't work and or you don't like it? Mm-Hmm . <affirmative> , um, you have bad hand function. It's creeping out again, your your , your lady partner or male partner, whatever strikes your fancy, he is gonna feel this thing. So you can't sneak a ring into the bedroom and hope it , they don't notice it. Um, you know, the before , uh, the next step would be needles. You know, you can guarantee yourself an erection inside an erection whenever you want it for as long as you want it. Hopefully less than four hours. But it's a needle in your penis whenever you want sex. Which kind of ruins the mood on Valentine's Day. But then again, you know, but you
Speaker 4Can Is it , isn't it true though , if I'm a man, I wanna do the trimix injection or whatever the injection is, the doctor prescribes the needle's super tiny. You , it doesn't hurt. That's right . Do I just excuse myself from my lady friend or man friend and go, I'm gonna go to the bathroom. Is it that quick? Or you do it real quick in the bathroom and get back
Speaker 6To bed? Absolutely . You can hide it much easier than a ring. Okay . You just have to look , I'm a wuss . I said, what would I want on my own penis? It's, it's not a , a shot to start with, but it , it , it is as close you can get to a guarantee as you can short of an implant, which we'll get to a second. Mm-Hmm. <affirmative> .
