SPEAKER_02Right. Okay, so before we can even begin to dissect how patients are being misled, we have to establish the baseline, right? Like what does the hard science actually say?

SPEAKER_00Right. Despite that, there is actually very limited high-quality clinical trial evidence comparing treatments.
SPEAKER_02Wait, really? That blows my mind. The research is sparse.
SPEAKER_00Extremely sparse. When it comes to comparing acute surgical reconstruction against non-surgical rehabilitation, there are primarily two major landmark RCTs.
SPEAKER_01Okay, what are they?
SPEAKER_00One is known as the compare trial, and the other is a famous foundational study led by a researcher named Frobel.
SPEAKER_02Okay, the compare trial and Frobel's work. What did these gold standard trials actually find? Because clearly it's causing some drama in the medical community.
SPEAKER_00They both arrived at the exact same paradigm-shifting conclusion. ACL reconstruction surgery is no more effective at improving patient-reported outcomes than initial management with rehabilitation alone.
SPEAKER_02Let's just um pause and hold on that for a second.
SPEAKER_00It's a lot to take in.
SPEAKER_02This surgery, the invasive, expensive, highly praised surgery does not produce better outcomes than just doing targeted physical therapy.
SPEAKER_00On average, across the population studied, no.

SPEAKER_00The outcomes for pain, symptoms, function in sports, and knee-related quality of life were fundamentally similar between the two groups. Wow. But it goes even deeper. Both trials revealed that if you take a group of patients with a fresh ACL tear and start them on a strict, high-quality rehab program, 50% of them will never require surgery.

SPEAKER_00Yes. Their knees stabilize, they regain function, and they return to their lives.
SPEAKER_02Okay, but what about the other 50%? The ones who try rehab, but their knees still feel, you know, unstable. Do they mess up their chances by waiting?
SPEAKER_00That's the beauty of the trial data. The evidence clearly shows that for those who try rehab first and later opt for delayed surgery, their long-term outcomes are just as good as the people who rushed into early surgery.
SPEAKER_02So you literally lose nothing by trying rehab first.
SPEAKER_00Nothing. Furthermore, from a healthy economic standpoint, early surgery is objectively not cost-effective.
SPEAKER_02Okay, we really need to unpack the how and why here, because this goes against everything we think we know about anatomy, right?
SPEAKER_00Aaron Powell It requires a big shift in how we view the body.
SPEAKER_02Yeah. Because most people view the body like a machine. If a timing belt snaps in your car's engine, you don't just drive carefully to compensate.
SPEAKER_00No, you replace the belt or the engine dies.
SPEAKER_02Exactly. If the ACL is a torn ligament, like a snap structural cable in the knee, how is it possible that half of people can just walk around and play sports without fixing it?
SPEAKER_00It's a great question. The human body is not an inert machine, it's a highly dynamic, adaptable, biological ecosystem. Let's use a different analogy. Imagine the knee is a suspension bridge.
SPEAKER_02Okay, I'm picturing the Golden Gate Bridge.
SPEAKER_00Perfect. The bones, your femur, and your tibia are the massive towers of the bridge. The ACL is one of the primary static internal cables holding those towers in alignment.
SPEAKER_02Right. It stops the tibia from sliding too far forward.
SPEAKER_00Exactly. When you tear the ACL, that specific internal cable snaps.
SPEAKER_02Which sounds like a structural catastrophe. I mean the bridge should collapse.
SPEAKER_00It would if that cable were the only thing supporting the bridge. But the knee has a massive active muscular support system built around it.
SPEAKER_02The hamstrings and quads.
SPEAKER_00Yes. Specifically the hamstring muscles in the back of your leg and the quadriceps in the front. These act as dynamic adjustable cables on the outside of the bridge.
SPEAKER_02Okay, I'm tracking with you.
SPEAKER_00Through intense specialized rehabilitation, you can train your hamstrings to fire instantly and forcefully whenever the knee is challenged.
SPEAKER_02Oh wow. So the muscles learn to act as a dynamic stabilizer.
SPEAKER_00Precisely. They actively prevent that forward slide of the bone, perfectly compensating for the missing static cable.
SPEAKER_02So you're basically rewiring the muscular response to take over the job of the torn ligament. That is incredible.
SPEAKER_00It really is. It's called neuromuscular control. Your nervous system and your muscles adapt to stabilize the joint. This is why 50% of people can function perfectly well without an ACL.
SPEAKER_02But I imagine this biological reality is incredibly difficult to explain to a patient who has just been handed an MRI showing a broken ligament.
SPEAKER_00It is extremely difficult. Which perfectly explains the survey data in this Phil-based study.
SPEAKER_02Right, because while the science says rehab is a completely viable, cost-effective first option, the public believes the exact opposite.
SPEAKER_00Strongly the opposite.
SPEAKER_02What were the actual numbers on patient beliefs from the study?
SPEAKER_00The misconceptions are deeply entrenched. The researchers found that 51% of patients firmly believe that surgery is an absolute requirement to return to any contact or pivoting sport.
SPEAKER_02Even though the data says that's false.
SPEAKER_00Completely false.
SPEAKER_02Yeah.
SPEAKER_00And even more concerning, 30% of the patients believe that if they chose to avoid surgery, they were practically guaranteeing they would develop severe osteoarthritis in their knee later in life.
SPEAKER_02Oh man. Let's talk about the arthritis myth, because that is the ultimate fear-mongering tactic, right?
SPEAKER_00It really is used that way.
SPEAKER_02Is there any truth to the idea that a surgically reconstructed knee is protected from arthritis?
SPEAKER_00None. The clinical data shows that undergoing ACL reconstruction surgery does not protect the knee against osteoarthritis.
SPEAKER_01Really? Not at all.
SPEAKER_00Not at all. In fact, if we look at the biological mechanisms of surgery, it might actually make things worse for some people.
SPEAKER_02Wait, how so? How does fixing the knee make arthritis worse?
SPEAKER_00Think about what a surgical reconstruction actually entails. It is massive biological trauma.
SPEAKER_02Yeah.
SPEAKER_00They drill wide tunnels straight through your tibia and femur bones.

SPEAKER_00It's terrible for it. You're altering the microscopic kinematics the exact way the joint glides and rotates. And we know that inflammatory joint trauma is a primary driver of osteoarthritis.
SPEAKER_02Aaron Powell Yet 30% of patients are walking into the clinic believing that going through this massive trauma is somehow going to prevent joint degeneration.

SPEAKER_02Okay, so we've established that the manufacturer's manual, the RCT data, says trying rehab first is highly effective, safe, and often bypasses the need for surgery entirely.
SPEAKER_00Correct.
SPEAKER_02So now let's pivot to the consultation room. Let's look at what the mechanics, the medical professionals are actually telling these patients in the real world.
SPEAKER_00This is where things get really concerning.
SPEAKER_02Aaron Powell Because if the science is this clear, you'd expect the doctors to be preaching rehab from the rooftops, right?
SPEAKER_00You would expect that.
SPEAKER_02Yeah.
SPEAKER_00But the quantitative data from the Phil-Base study reveals a clinical landscape that is actively hostile to the scientific consensus.

SPEAKER_00Out of the 734 participants, an overwhelming 74% ended up getting the ACL reconstruction surgery.

SPEAKER_02Exactly. Only 16% manage their injury with rehabilitation alone.
SPEAKER_00That is a massive disconnect. So the immediate question is: who are these patients talking to that is funneling them so aggressively onto the operating table?
SPEAKER_02The patients consulted a wide multidisciplinary net. 94% saw an orthopedic surgeon, 91% saw a physiotherapist, and 65% saw a general practitioner, a GP.
SPEAKER_00Okay, so they're seeing all the right people. What were these professionals telling them?
SPEAKER_02The survey data is damning. 85% of orthopedic surgeons, 65% of GPs, and 61% of physiotherapists presented surgery as the best treatment option for the patient.
SPEAKER_00Wait, the best option? Not just an equal option?
SPEAKER_02Not equal. The definitively superior option. And when the topic was returning to competitive sports, 84% of surgeons and 62% of physios told patients surgery was the only real way to get back on the field.
SPEAKER_00Hold on, let me just um process this. You just told me the two largest clinical trials in the history of this injury found no meaningful difference in return to sport rates. That's right.
SPEAKER_02Yet 85% of surgeons are looking their patients in the eye and telling them surgery is the best option. Are they just lying?
SPEAKER_00Well, let's look at the most damning statistic of all. Only 10% of orthopedic surgeons.
SPEAKER_02Wait, 10%?
SPEAKER_00Yes. Only 10% of surgeons and 29% of physiotherapists accurately inform their patients that the outcomes are, on average, similar between surgery and rehabilitation.
SPEAKER_02One in ten surgeons is actually quoting the data accurately. That is terrifying.
SPEAKER_00And it gets worse. 39% of surgeons only discuss surgery as an option. They completely omitted rehab from the conversation entirely.
SPEAKER_02Completely omitted it. I mean, that is the definition of clinical malpractice from an informed consent standpoint.

SPEAKER_02I have to push back here. Hard. Because this borders on unbelievable. You're telling me that a highly educated surgeon, whose literal professional duty requires staying up to date on best practices, is just ignoring the two largest trials in their field.

SPEAKER_02Why? Are they willfully ignoring it to book more lucrative surgeries? Or do they genuinely believe they know better than the scientists?
SPEAKER_00I'm sorry, but if a structural engineer ignored the data on how much weight a bridge could hold, we'd call it negligence. We would. So why do surgeons get a pass?
SPEAKER_02Your frustration is entirely justified, and it cuts to the core of how medical culture operates. To understand the why, we have to look at the psychology of the surgeon and the concept of clinical bias.
SPEAKER_00Okay, let's hear it. Is it financially motivated?
SPEAKER_02In a private fee-for-service healthcare system, we cannot ignore the reality that operating pays significantly more than recommending physical therapy. A surgical booking is highly lucrative.
SPEAKER_00Follow the money, right? It's age-old story.
SPEAKER_02Yes, but it is rarely a conscious, malicious plot to defraud the patient. It's far more insidious than that. It is deep-seated cognitive dissonance.
SPEAKER_00Cognitive dissonance? How so? Surgeons are trained for years in surgical technique. Their primary tool is the scalpel. When a patient comes to them with a torn ligament, they look at the MRI, they see a mechanical defect, and their entire training screams, I know how to fix this mechanically. Precisely. And remember, they see the immediate mechanical success of their work. They rebuild the ligament, the knee feels structurally tight on the operating table.
SPEAKER_02And then they see the patient walking a few months later.
SPEAKER_00Right. They internalize that as a profound clinical success. So when a researcher knocks on their door and says, actually, a spreadsheet of RCT data shows the patient would have been fine without you.
SPEAKER_02It threatens their entire professional identity.
SPEAKER_00Exactly. It is incredibly difficult for human beings, even highly intelligent ones, to accept that an external trial invalidates their daily tactile reality.
SPEAKER_02That makes sense psychologically, but regardless of their internal psychology, the consequence for the patient is catastrophic because it completely destroys what the study calls shared decision making.
SPEAKER_00Yes. Let's define that because it's vital.
SPEAKER_02Go for it.
SPEAKER_00Shared decision making is the ethical cornerstone of modern medicine. It is defined as a collaborative process where clinicians and patients share the best available scientific evidence, and the patient is supported to consider all options to achieve an informed preference.
SPEAKER_02Key words there being best available evidence and all options.
SPEAKER_00Exactly. For shared decision making to exist, the clinician must act as a neutral broker of scientific truth. Which they aren't doing. If 39% of surgeons are refusing to even mention rehabilitation, shared decision making is dead on arrival.
SPEAKER_02It's like if you walk into a car dealership and the salesperson hides every car except the most expensive model and tells you it's the only one that runs, you aren't making a choice.
SPEAKER_00No, you are being manipulated. A patient cannot give truly informed consent for an invasive surgery if they are actively kept in the dark about a non-surgical alternative that has a 50% success rate.
SPEAKER_02Man. So since these surgeons are clearly the loudest, most authoritative voices pushing for the operating table, we need to zoom in on the patient's actual lived experience.
SPEAKER_00We do. We need to look at what it feels like to sit across from them.
SPEAKER_02Right. And the Phil-Base study didn't just crunch numbers, they conducted these 12 in-depth qualitative interviews. They basically gave these patients a microphone.
SPEAKER_00And this is where the sterile statistics take on a very real, very distressing human pulse. When you analyze the transcripts of these interviews, very distinct themes emerge about the power dynamics of that room.
SPEAKER_02The first major theme was rushed consults. We were talking about 15-minute appointments to decide the fate of a person's physical mobility.

SPEAKER_02There was a participant named William. His quote is just haunting. He said, This is the most I can tell you in 15 minutes, and see you later.
SPEAKER_00Think about the psychological asymmetry of that room. You are the patient. Your life has just been abruptly derailed.

SPEAKER_00You're in pain. You're worried about your job, your hobbies, your identity as an athlete or just an active person. You are desperate for guidance.
SPEAKER_02And the person sitting across from you, the person with all the answers, is glancing at the clock, rushing through an explanation, and handing you surgical consent forms.
SPEAKER_00It's a conveyor belt.
SPEAKER_02It really is. Another participant, Sandra, described her experience with her surgeon as borderline traumatic.
SPEAKER_00Her story was particularly rough.
SPEAKER_02The surgeon walked in, used highly catastrophic language, saying her knee was basically exploded, told her definitively she would never play sports again without surgery, booked her in for the following Wednesday, and then just walked out.
SPEAKER_00He didn't even provide basic educational materials.

SPEAKER_02The aggressive downplaying of surgical risks, right?
SPEAKER_00Yes. Sandra noted that the surgeon casually brushed the procedure off as just a little keyhole surgery.

SPEAKER_00The use of diminutives like little is a classic linguistic tactic to minimize patient anxiety and streamline the booking process.
SPEAKER_02Because it sounds so minor.
SPEAKER_00But as we discussed earlier, ACL reconstruction is not a minor procedure. It is a major orthopedic trauma. You are harvesting healthy tissue from elsewhere in the body, drilling into the skeletal structure, and initiating a massive inflammatory cascade.
SPEAKER_02Calling it a little keyhole surgery makes it sound like they're just popping into change of blown fuse, but they're literally ripping out pieces of your hamstring.
SPEAKER_00By portraying it as a trivial, guaranteed fix, clinicians set the patient up for severe psychological shock when the reality of the recovery sets in.
SPEAKER_02Because they routinely fail to mention the dark side of the statistics.
SPEAKER_00Exactly. They omit the grueling, painful months of post-operative rehab, the very real risk of infection or blood clots, the fact that a significant percentage of patients never actually return to their pre-injury level of sport.
SPEAKER_02And perhaps most terrifying, the risk that the new surgical graft might simply tear again.
SPEAKER_00Which happens more often than people realize.
SPEAKER_02Which brings us to the absolute tragedy of a participant named Leslie. If any story encapsulates the failure of the system, it is his. But nobody prepared him for the reality of surgical recovery. Not at all. He describes waking up from the surgery crying in sheer agony. His knee was completely stiff, heavily bandaged, his leg immobilized.
SPEAKER_00Which is a huge shock.
SPEAKER_02He had literally been running, albeit with a torn ACL, the day before the surgery, and suddenly he was a total invalid. But the trauma didn't stop at the recovery room, did it?
SPEAKER_00No, it gets so much worse. Leslie grinds through the rehab, he finally gets back out on the field, and the worst case scenario happens.
SPEAKER_02He ruptures the surgical graft, the new ACL tears.
SPEAKER_00So he goes back to the exact same surgeon, the same guy who sold him the dream of returning to sports.
SPEAKER_02And what does the surgeon say to him now? He looks at Leslie and says, Mate, you're done. You've got degenerative arthritis, your cartilage is shot. If you want to have any quality of life later on, hang up the boots.
SPEAKER_00The devastating finality of that statement is chilling. It is a total abdication of the promises made during the initial consult.
SPEAKER_02This is what I really want to highlight. The emotional whiplash is psychotic.
SPEAKER_00It really is.
SPEAKER_02The surgeon initially sells you this dream, portraying surgery as a magical time machine that will rewind your knee to its factory settings. But when the statistical reality of a graft failure hits, which the RCTs clearly show is a substantial risk, that exact same surgeon suddenly flips the script.
SPEAKER_00Suddenly he acts like your athletic life was always doomed.
SPEAKER_02Exactly. How on earth does a medical professional justify that pivot?
SPEAKER_00It reveals a profound failure to manage the psychology of expectations. Studies have shown that patients go into ACL reconstruction with wildly, dangerously unrealistic expectations.
SPEAKER_01Like what?
SPEAKER_00Surveys indicate that nearly 100% of patients expect to have a completely normal or nearly normal knee after surgery. Over 90% expect to return to their pre-injury level of sports without limitations. They expect zero increased risk of arthritis.
SPEAKER_01Well, yeah, because that is exactly what the rushed 15 minute consult implied.
SPEAKER_00Exactly. Because surgeons fail to temper these expectations with hard data, patients build their entire psychological recovery around a myth.
SPEAKER_02They aren't educated on the nuances of joint kinematics or the statistical realities of re injury.
SPEAKER_00So they accept the Quick fix. But when the surgery isn't a magic bullet, when the graft fails, or the chronic pain sets in the patient is left psychologically shattered.

SPEAKER_02But they're not the only doctors in the ecosystem. Where are the checks and balances? Why aren't the general practitioners or the physiotherapists the people who specialize in non-surgical movement pumping the brakes and waving the compare trial data in the patient's face?
SPEAKER_00The breakdown of the supporting cast is one of the most fascinating sociological findings of the Phil-Bay study.
SPEAKER_02Let's look at the general practitioners first.
SPEAKER_00In most healthcare systems, including Australia, where the study was based, the GP is the primary point of contact. They're supposed to be the holistic guide.
SPEAKER_02Right, the gatekeeper in a good way.
SPEAKER_00But the qualitative interviews reveal that patients view GPs purely as bureaucratic hurdles. They are administrative gateways to get an MRI referral or a surgeon's phone number.
SPEAKER_02Yeah, the quotes from patients about their GPs were brutal. A participant named Kelly bluntly stated, I find GPs pretty useless with this stuff. I basically just walk in and tell them the problem and make them write what I want in the referral.
SPEAKER_00Patients simply don't expect their GP to have specialized, up-to-date knowledge on orthopedic clinical trials.
SPEAKER_02And alarmingly, the data suggests the GPs agree with that assessment.
